Birthday Book Club Form 2012- 2013 (due date extended to September 7, 2012)

 

 

Celebrate your child’s birthday with the SPARK Media Center by purchasing a book in their honor. For a donation of $20.00 they will have the opportunity to choose a book from our special Birthday Batch. A bookplate will be placed in the book and your child will be the first one to check out the book. When the book is returned it will be placed into circulation for all to enjoy.

 

To participate, just fill in the form below and send it to school along with a check for $20.00 made out to the SPARK PTO.
